Grant Application

The application is a two step process:

1) Completing and Submitting the Application

2) Emailing the required documents to teamhussain007@gmail.com with "Ummah Relief- COVID-19 Grant" in the subject line.

 

All applications must be accompanied with:

  1. Photocopy of Photo ID (Drivers license or passport)

  2. Proof of financial standing (Previous 2 months of Bank Statements)

  3. Proof of address (Utility bill)

  4. If Married, Spouse's Photocopy of Photo ID (Driving license or passport)

Applications missing required documents will be considered incomplete, will not be reviewed, and/or considered even if they meet the selection criteria.

Due to the anticipated number of applications, we will not be able to communicate with all applicants to inform them of their status. Successful applicants will be contacted within three 3-5 days of their submission.
